You most likely have a corrupt document and/or attachment. This should definitely get you moving:
- Create a new copy of the database (File -> Database -> New Copy)
- Backup and delete all of your old copies and replicas
- Put the copy in place of the old database
- Create the FT index
If that works, re-create your replicas and you should be running smoothly.
